Análisis retrospectivo de biomarcadores asociados con la mortalidad en pacientes con COVID-19

Resumen

El virus SARS-CoV-2 causa manifestaciones respiratorias que varían desde cuadros leves hasta neumonía grave. Los pacientes críticos pueden presentar múltiples complicaciones y requerir ventilación mecánica invasiva, lo que incrementa el riesgo de mortalidad. La identificación de factores de riesgo de muerte, modificables, nos permite tomar decisiones terapéuticas oportunas. El objetivo de este estudio retrospectivo fue determinar si la elevación del dímero D al ingreso en la Unidad de Cuidados Intensivos (UCI) constituye un factor de riesgo de mortalidad en pacientes críticos con neumonía por SARS-CoV-2. El estudio se realizó en una unidad de cuidados intensivos de segundo nivel de la ciudad de Quito-Ecuador. Se incluyó a todos los pacientes mayores de 18 años con diagnóstico de neumonía por SARS-CoV-2 que ingresaron a la unidad y contaban con un resultado inicial de dímero D. Se recolectaron variables demográficas, clínicas, de laboratorio, de estudios de imagen, desenlaces y tratamiento. Los niveles de creatinina sérica fueron significativamente mayores en los pacientes fallecidos y aquellos con edad avanzada. Estos hallazgos sugieren una posible asociación entre lesión renal aguda e incremento de la mortalidad en pacientes críticos. Además, se recomienda evaluar el dímero D en una cohorte más amplia, con el fin de alcanzar una potencia estadística suficiente que permita contrastar los hallazgos reportados por otros autores.
